When will I receive my re-loadable debit card?
Once you have completed your latest research task the B3 team will mail you a paper disclosure form required by US Bank that you receive before the re-loadable debit card. After the disclosure is received a re-loadable debit card will be mailed to you with the task payment. This whole process usually takes between 2-3 weeks.
After I receive my card how do I activate it?
Call cardholder services at 888-863-0681. To activate you will need your birth date and the 16-digit number on the card.
Why can’t I create an account on US Bank for my re-loadable debit card?
Cardholders must be 16 years old or older to create an account on usbankfocus.com. You do not need to create an account to use or activate your card, once card is activated, to check your card balance call 888-863-0681.
How can I check my balance?
Call cardholder services at 1-888-863-0681. You can also visit your nearest US Bank ATM to check your balance. You can also check your balance at any non-network ATM, but you will be charged a $1.00 fee.
Can I withdraw my money from my re-loadable gift card?
Yes! You can withdraw your funds as cash from any US Bank ATM free of charge. You can also withdraw your funds from a non-network ATM, but there will be a $1.75 withdraw fee.
I’ve completed tasks, but I haven’t received a gift card.
If you joined the study before November 2023, we were having issues with our re-loadable debit card account. Payments went out as Amazon gift cards. If you haven’t received an Amazon gift card or re-loadable debit card, please contact us at B3@pediatrics.wisc.edu or text or call us at 608-218-4105. Please note that payments may take up to 3 weeks to be loaded on re-loadable debit cards.
I need a replacement card?
One free card replacement is included every 12 months. If you need more than one card replacement in 12 months US Bank will charge a $5 fee for every additional card needed within a 12 month period. To get a card replacement, please contact US Bank directly at 1-877-474-0010, US Bank will verify your mailing address and reissue a card within the next 7-10 business days.
Will I be charged a fee for not using my card?
Yes, there is a $2.00 fee charged after 1 year of non use. This $2.00 fee will be charged each month after you have not completed a transaction using your card for 365 consecutive days.
